Community outreach aims to provide a range of support to help individuals with a learning disability to access the community. At the same time, we work towards supporting individuals to achieve short and longer-term goals. Support is structured and covers various life skills, including:
managing health and well-being
accessing work or training opportunities
accessing local community groups
Whether someone requires a few hours of outreach support each week or an intensive period to work towards a specific objective, our experienced staff will support people to achieve their goals. As the members we support to develop their skills, we adapt our services to meet their changing ambitions. How we work with you: We will work with you to develop a ‘person-centred plan’ that will look in more detail as to what you would like to achieve. We then create an activity plan which provides details of what support you may benefit from and when the best time is to work with you. All outreach activities are focused on achieving positive outcomes for individuals. We work together to design and deliver support that promotes greater independence, confidence, and self-esteem. The important thing to remember when we are working with you on what you want to achieve is that we work to your schedule, not ours. How long support lasts: It really depends on you and how much support you feel you need to achieve your personal goals and outcomes. We promote and encourage self-development which means we focus on enabling you to build on your existing skills and move towards a more independent way of living that you are happy and confident with.
